Enrollment

In order to attend classes at The Learning Center, the following requirements must be met:

1) The child must be 3 or 4 years of age by August 31.
2) The child must be toilet trained and able to go to thebathroom independently.
3) The child must have a recent physical and must havereceived all the required immunizations.

The set of enrollment forms should be completed and returned to the Director by the first day of school. The health assessment form must be completed and signed by your child's physician or the health department. If it is impossible to complete the form by your child's first day of school, it is required for state licensing to notify the Director of the date of the doctor's appointment at which the physical will be completed. This information is required for the child's file. The ar)sentee consent form and the emergency medical form must be completely filled out and notarized! These two forms are due no later than the first day of school.
